Forms
Individuals have the right to request access to information under The Freedom of Information and Protection of Privacy Act (FOIP). FOIP allows access to records in the possession or under the control of eHealth Saskatchewan.
To request records held by eHealth Saskatchewan, complete the Access to information request form and submit it by email to privacyandaccess@ehealthsask.ca. When completing the form, please provide as much detail as possible. The more information you include, the better we can respond to your request.
To request your personal health information or medical records, please see our Access your personal health information page.
Under subsection 7(2) of FOIP, a government institution must respond to an access to information request within 30 days of receiving request.
You can also contact the eHealth Privacy Service for information, guidance or support. If you are unsatisfied with the service you received, you can contact the Office of the Saskatchewan Information and Privacy Commissioner, who oversees The Health Information Protection Act and The Freedom of Information and Protection of Privacy Act.
