Birth certificates

Who can apply

You can apply for a birth certificate or copy of birth registration if you are:  

  • the person named on the certificate, if you’re 15 or older 
  • a parent listed on the original birth registration if the child is under 18 
  • a legal custodian, personal guardian or property guardian

  • a personal representative of an eligible person’s estate 
  • applying to prove Métis or Treaty Status 
  • acting in the place of a parent, such as: 
    • the Public Guardian and Trustee
    • Social Services
    • First Nations Child and Family Service Agencies
  • named in a Saskatchewan court order or subpoena
  • authorized in writing by an eligible person

Types of certificates

Long form birth certificate

Certificate size: 5" x 7"

A long form birth certificate includes:

  • full name
  • birth date
  • place of birth
  • sex
  • registration number and date
  • parents' full names
  • parents’ places of birth

This is the best choice for children 16 and younger. Government agencies and schools often require a long form birth certificate. 


Short form birth certificate

Certificate size: 5" x 7"

A short form birth certificate includes:

  • full name
  • birth date
  • place of birth
  • sex
  • registration number and date

Copy of registration of live birth

Certificate size: 8.5" x 14"

This is a copy of the original legal birth registration. It includes the information recorded when the birth was registered.

It’s commonly used for international purposes. It can’t be used as personal identification. 


Genealogical copy

A genealogical copy is a copy of the original registration of birth used for family history research.

It’s available for births registered in Saskatchewan more than 100 years ago.

What you need to apply

To apply, you'll need:

  • your name, mailing address and phone number

  • the name, birth date and place of birth for the person named on the certificate

  • details about the person’s parents

  • one piece of government-issued ID, or two other pieces of ID with your signature 

You may need to provide more documents, depending on who is ordering the birth certificate.
