Requesting access to another health record

Children under age 14

Parents and legal guardians can request access to health information in MySaskHealthRecord for children under age 14.  

To request access to your child’s health information, all of the following must apply: 

• Your child is under age 14
• Your child has a valid Saskatchewan health card
• You have your own MySaskHealthRecord account
• You can provide proof of parentage or guardianship
• You complete and submit the request form 

Someone 14 or older 

Residents age 14 and over must create and manage their own MySaskHealthRecord accounts using a free non-driver photo ID from SGI. Parents will no longer have access. Users may choose to share their health information with others using the MySaskHealthRecord sharing feature. For more instructions on sharing records, please visit Share your health information.

A person aged 14 or older who lacks legal decision-making capacity 

Anyone legally responsible for the ongoing care of someone who lacks capacity and is not expected to regain it may request access to that person’s MySaskHealthRecord.  

Examples include: 

• personal guardians
• substitute health care decision-makers
