Chronic Disease Management – Quality Improvement Program
The Chronic Disease Management – Quality Improvement Program (CDM-QIP) gives health care providers tools to improve follow-ups and provide better care for people living with chronic diseases.

CDM-QIP supports care for:
- diabetes (DM)
- coronary artery disease (CAD)
- heart failure (HF)
- chronic obstructive pulmonary disease (COPD)
Chronic disease information is collected and stored in the Electronic Health Record (eHR) Viewer. This may include:
- blood pressure
- smoking status
- height and weight
- certain laboratory results
Program partners
CDM‑QIP is a collaboration between the Ministry of Health, the Saskatchewan Medical Association (SMA) and eHealth Saskatchewan:
- The Ministry of Health leads physician payment.
- The SMA manages clinical information.
- eHealth and the SMA share responsibility for delivering the service and protecting the privacy of program information.
- eHealth provides secure electronic systems for collecting data, reporting and improving care while protecting personal information and personal health information.
Program goals
The program goals are to:
- improve the continuity and quality of care for people living with chronic conditions
- encourage physicians and other health care providers to use best practices
- use Saskatchewan’s health information system to better meet the needs of residents and providers
