Certified criminal record check: step-by-step guide

If you are 18 years or older and applying for a legal change of name, you must submit an original certified criminal record check with your application.  

1. Choose where to get fingerprinted

Your fingerprints must be taken in Saskatchewan by one of the following: 

• your local police service 
• an RCMP detachment that offers fingerprinting 
• an RCMP‑accredited private fingerprinting company 

A list of accredited private companies is available on the RCMP website.  
 

You may need to book an appointment. Fees vary by agency. 

2. Bring the right information

When you have your fingerprints taken, provide the information below exactly as shown. 

• Application type: 
  Select: Other (Specify Reason) 
• Application details: 
  Enter: Name Change – Saskatchewan 
• Contributor type: 
  Select: Individual 
• Current legal name: 
  Enter your last name, first name and middle name 
• Proposed new name: 
  Enter your last name, first name and middle name 
• Send results to 
  Select: Individual 
• Mailing address: 
  Use your home address. 

Your mailing address must match your home address. If you need your results mailed to a different address, such as a post office box, contact the RCMP Canadian Criminal Real Time Identification Services (CCRTIS):

• Hours: Monday to Friday, 7:30 a.m. to 5:30 p.m. Eastern Time 

3. Avoid common mistakes

Your application will be rejected if the information is entered incorrectly. 

For application type, do not select: 

• Name Change – Saskatchewan Criminal Record Check for Personal Use – Other 

For any required field, do not enter: 

• Unknown 
• X or XXX 
• N/A 

All fields must be completed correctly. 

4. Receive your results

The RCMP’s Canadian Criminal Real Time Identification Services (CCRTIS) will mail the results directly to you. 

You will receive an original paper copy.  

5. Submit your results

Submit the original certified criminal record check with your legal change of name application. 

eHealth must receive the results within 14 days of the date shown on the document.

Copies are not accepted. 

6. Pay required fees

You pay fingerprinting and certified criminal record check fees directly to the agency that provides the service.  

Fees vary by agency. 

If your legal change of name application is denied, eHealth Saskatchewan doesn't reimburse fingerprinting or certified criminal record costs.
